Inpatient Service
With several ensuite residences and private facilities, we offer short-term admission to support patients with acute and/or complex specialist palliative care needs.

Home Care
If life-limiting patients opt to remain at home, our well-trained team of healthcare professionals will support through the 24-hour hotline and conduct home visits on an as needed basis.

Day Service
Our day service sees to a patient’s needs away from home and assist in symptom reliefs. Here, patients also interact with others and engage in constructive activities.

Psychosocial and Spiritual Support
Both psychological and spiritual needs of patients and families can be just as important as other needs at the end of life.

Complementary Therapies
Complementary therapies and allied support such as Physiotherapy and Occupational Therapy, Aroma Therapy, Art or Music Therapy are offered to improve quality of life of patients and carers.

  • Natural Ambience - JCHH perches on a hillside surrounded by a mindful greenery in natural ambience serving as a high-privacy place of respite during the course of illness and a final stop for life-limiting patients
  • Holistic Professional Care - Our multi-disciplinary team of medical, nursing, allied health professionals from different disciplines provides all-round services to our patients and their families

Service Approach

  • Family-based Approach - Together with the patient, we encourage family members and caregivers are involved, in discussions about their condition, prognosis and care plan, which is crucial for the patients to walk through their last stage of life with their loved ones
  • Dedicated Follow-up Service - Customized holistic care plans to best fit patients’ needs and a dedicated case manager for the seamless offering of services by various disciplines, volunteers and caregivers

Who needs JCHH’s service?

Patient who are diagnosed with an incurable disease and are considered to be approaching the end of life but wish to have good symptom control, quality care, and stay in the comfort place or their own homes with the company of their families. This includes people whose death is imminent, as well as people who:

  • have advanced incurable illness such as advanced cancer, end-stage organ failure, and degenerative illness;
  • are generally frail and have co-existing conditions that are expected to pass away soon

Care Team

Services of JCHH are delivered by a diverse team specializing in hospice and palliative care. This multi-disciplinary team of professionals consists of physicians, nurses, physiotherapist, occupational therapists, counselling psychologists, social workers, pastoral care workers, personal care workers and other healthcare staff working in close collaboration with trained volunteers, caregivers and family members

Thanks to the HKSAR Government and The Hong Kong Jockey Club Charities Trust respectively for granting a piece of land and funding the cost of building, construction of JCHH commenced in 2013 and completed in mid 2016 with inpatient care services starting in late 2016.
