We respect the choice of life-limiting patients. If they opt to remain at home, our dedicated, well-trained team of healthcare professionals will support and conduct home visits on an as needed basis and work with patients and family members throughout this process. If needed, access to hospice and palliative care is available at home through the 24-hour hotline.
If a patient has an illness that cannot be cured, palliative care allows the patient to be as comfortable as possible by managing his/her pain and other distressing symptoms while having time to create their legacy and reflect on life. Specifically, the team provides for:
- Symptom management
- Drug supervision
- Nursing procedures
- Referrals for allied support
- Psycho-social spiritual support
- Bridging community resources
- Equipment and aids
- Caregiver education
- Assessment of empowerment of caregiver
- Provide holistic support to people who opt to die at home
- Bereavement support

Compared to patients who did not receive palliative care, local and overseas studies have indicated that palliative home care has achieved positive outcomes in symptom control; less invasive treatments during the last two weeks of life; shorter stay in acute hospital settings, less admissions to intensive care unit in addition to satisfaction by both patients and the caregivers.
Our hospice team trains family members or caregivers so that they are equipped with the necessary skills to care for the patient between our visits. If help is needed, assistance is just a phone call away 24 hours a day.
Yes. In fact, hospice physicians and nurses consult with the patient’s doctor on all aspects of his/her care.
JCHH ensures that no patient will be deprived of appropriate hospice and palliative care; thus financial assistance in the form of fee reduction can be offered to patients on an as needed basis fulfilling certain criteria and the completion of a form.
