The Jockey Club Home for Hospice (JCHH), established by the Society for the Promotion of Hospice Care (SPHC), is a hospice centre that brings forth a new era in caring for patients with life‐limiting illness, ensuring patients to make the most of their remaining days with dignity and reduced suffering.

JCHH is a home from home. Emotional, psychological, spiritual and practical support and care are provided for by an inter-disciplinary team and offered to patients with life‐limiting illness and families through a plan tailored for each family unit.

  • Brand new hospice centre in a natural setting with a high degree of privacy– serves as a respite during the course of illness and a final stop for life-limiting patients
  • Comprehensive and holistic– with an inter-disciplinary team of medical and healthcare professionals providing all-embracing services looking after the body, mind, spirit and soul
  • Family-oriented– involvement of caregivers is crucial as we recognize the impact of life-limiting illness and death on the patients as well as their loved ones
  • Case management– customized holistic care plans to best fit patients’ needs and a dedicated care manager for the seamless offering of services by various disciplines, volunteers and caregivers

Adults, young people or children diagnosed with an incurable disease but wish to stay in the comfort of their own homes with the company of the families.

Although this isn’t always possible to predict, people are considered to be approaching the end of life when they are likely to pass away within the next 6 months. This includes people whose death is imminent, as well as people who:

  • have an advanced incurable illness such as cancer and terminal renal failure
  • are generally frail and have co-existing conditions that mean they are expected to pass away within 6 months
  • have existing conditions if they are at risk of dying from a sudden crisis in their condition
  • have a life-threatening acute condition caused by a sudden catastrophic event, such as an accident or stroke

Services of JCHH are delivered by a diverse team specializing in hospice and palliative care. This inter-disciplinary team of professionals consists of physicians, nurses, physio or occupational therapists, clinical psychologists, social workers, personal care workers and other healthcare staff working in close collaboration with trained volunteers and family members.
