Palliative home care is a specialized medical service for people with any advanced illness or life-limiting condition who wish to remain at home. As our home care patient, you will be assigned a case manager who is an experienced palliative care nurse and also your main point of contact to the rest of your health care team. Your case manager will visit you regularly to monitor your care and ensure that your physical and psychosocial needs are being met.

Palliative home care is suitable for patients who are in the early stages of the disease process and are undergoing curative treatments. Patients with life-limiting illnesses who seek out palliative home care early stand to benefit from it the most.

Home care doesn’t mean you have to be at home to receive our care. We are more than happy to visit patients at aged homes or hospitals. If you are an existing in-patient at the Jockey Club Home for the Hospice, you are welcome to approach one of our in-patient nurses to learn more about our home care services.
